Commonly Affected Parts in Lichen Planus

Conium maculatum:

This drug is prepared from the plant commonly known as poison hemlock. Acts on the glandular system, engorging and indurating it, altering its structure like scrofulous and cancerous conditions. Sabal Serrulata:

Sulphur:

Anatomy of Tonsils: The structure:

The tonsils consist of two glandular tissues similar to hundreds of other lymph glands in the human body, present on either side of the back side of the throat. They are normal body organs just like lungs, skin and eyes. They are visible just behind the tongue, when the mouth is opened with slight force. They may vary in size. They are soft to touch and may either be bright red or pink-grayish in color.

Physiology of Tonsils: The Functions:

Tonsils, like any other body organ, have a specific role to play. Rather, tonsils have been allotted the vital task of checking if there are certain intruders such as bacteria, virus, allergens, etc attempting to attack the human body. Tonsils being lymphoid tissues, they produce the special cells called Lymphocytes. The Lymphocytes are similar to the white blood cells (Leukocytes) with which they form an immunological network of self-defense.

Immunology of Tonsils: The Inner Power:

As said earlier, the tonsils work as a filter which fights and protects the entire human system against the foreign organisms. They also help preventing spread of infection from the nearby organisms such as mouth, sinuses, postnasal part, etc. Tonsils produce antibodies, which fight against the infection, stopping its further spread to other parts of the body. When the bacteria or virus attack the body, they initially have to face the tonsils. In the process of fighting towards the germs and microbes the tonsils get inflamed (called tonsillitis which is simply a symbol of the local defense mechanism at work. In the process, they produce lymphocytes, and antibodies to generate the required immune response. When you see the tonsil infected, inflamed and swollen, actually you are seeing the tonsils at work. However, it is not implied that the inflamed tonsils do not need treatment. They do need treatment with a scientific attitude. They need to be treated but not to be necessarily removed! Compare them again with the soldiers fighting at the battle field, who need special care but not removal from the battle field unless they are seriously injured or incapable of doing their job!

Tonsillitis: Causes

The commonest cause of tonsillitis is infection of the tonsils by micro-organisms. Certain bacteria (most common being the streptococcal group of bacteria), viruses, and allergens are known to produce tonsillitis. Allergens are usually in the form of certain food articles. Common food articles that can trigger tonsillitis are artificially colored sweets, sour fruits, bananas, preservatives added to certain drinks, cold food or drinks, ice-creams, etc. Environmental factors that may trigger an attack of tonsillitis are exposure to excessive cold weather, damp climate or change of weather. It must be borne in mind that bacteria and viruses tend to flourish in crowded areas and hence patients who are prone to tonsillitis can easily catch the infection in schools, parks, theatres, etc. All these factors contribute to lowering the immunity of the body and thus the tonsils fall prey to infection by the micro-organisms. Another causative factor that can be of importance in the diagnosis of recurrent tonsillitis or chronic tonsillitis is genetic tendency. Recurrent tonsillitis is frequent in patients whose parents have also suffered from the same condition during childhood or adolescence. Thus it is not just one, but multiple factors that combine together to lower the immunity of the body and this in turn causes recurrent and chronic tonsillitis in patients.

Tonsillitis: Symptoms

01 Sore throat: Pain in the throat is the most common presentation. However, young babies may not present with the pain but may present with an inability to eat. 02 Dysphagia: That is difficult swallowing. This may either be due to pain or due to huge increase in the size of the tonsilsdue to frequent inflammation. 03 Fever: Acute infection of the tonsils may present with moderate to high rise in body temperature. In case of septic foci on the tonsils, there may be fever with chills. 04 Apnea : Sleep apnea, a disturbed sleep disorder may occur due to an obstruction to the air passage due to heavily enlarged tonsils or adenoids. However, it is interesting to note that most children with sleep apnea do not recover of this disorder after the removal of the tonsils. (This is because, in many cases, there are associated causes for apnea, such as obesity, adenoids, small jaw, bony skull deformity (congenital), neuromuscular disorder, etc.) 05 Snoring: Due to obstructed air passage caused by the enlarged tonsils, snoring may be one of the symptoms in many children suffering from recurrent tonsillitis. Again, it may be noted here that snoring may be due to reasons other than the enlarged tonsils. 06 General symptomsTonsillitis may be associated with general fatigue, tired feeling, bodyache, loss of appetite, low energy level, headache, etc. 07 On examination: The attending doctor may have a look at the throat with a torch to find inflamed, red, congested tonsils. There may be visible septic foci (small pus pockets). Externally, one may palpate enlarged and mildly painful neck glands, indicating some more lymph glands in the neighborhood.

China is frequently used in the treatment of neuralgic pains, anemia, hemorrhages, debilitating illnesses, fevers, dyspepsia, asthma, etc. To describe a few of the important symptoms of China: There is marked debility in the patient due to the loss of vital fluids. Platina:

Predominantly affects the nervous system and the female reproductive system. It has a special affinity for Trigeminal, vagus, and sensory nerves. Some of the common indications for Platina are given below: Violent, cramping, numbing pains, later progressing to spasms. Painful tremors and spasms. Facial pain on the right side that extends in all directions and is accompanied by redness and burning of the cheeks.

Aranea diadema:

This is a wonderful remedy for Trigeminal neuralgia and has been found very useful in many cases of TN. The patient gets neuralgic pains which are worse in a damp climate and worse in cold weather. The pain gets worse around afternoon and at midnight and is better by pressure. The pain returns periodically i.e. the neuralgic attacks occur at the same hour every day, every other day, week, month or at regular intervals. Violent, sudden pains like electric shocks. Pain in the right trigeminal nerve from periphery inwards. Sudden violent pain in teeth at night immediately after lying down. Sensation as of enormous enlargement or of numbness of parts.

Verbascum:

This homeopathic remedy is prepared from Mullein Oil and is one of the very good remedies for Trigeminal neuralgia. It is commonly used for colds and catarrhs, spasmodic effects and neuralgic pains. What is Urticaria?

Urticaria also called nettle-rash or hives or wheels in a common language, simply means itching with a rash. Medically, urticaria may be defined as skin eruption, which is allergic (or non-allergic) in origin and is characterized by profound itching, red circular or irregularly shaped eruptions on any part of the body. Urticaria is commonly misspelled as uticaria, utricaria, urticara, uricaria. Urticaria is called as pitti in Hindi. Urticaria is an allergic or non-allergic immunological disease, shown on the skin. Characteristically the skin eruptions are erythematous, raised above the skin level, with intense itching and usually worsened by itching an with slight local warmth. It can be acute or chronic; largely having a tendency to recur frequently for many months or years. These eruptions can remain on the body for a variable period anywhere between few seconds to even hours. They have a tendency to disappear and reappear. They tend to disappear without leaving behind any trace. Location and duration: Well, urticaria may appear on any part of the skin. Angioedema is a condition when deep tissues are affected. The typical lesions may last for one minute to half an hour. Some may last even longer. Some patients may get the eruptions once in a while and some may have many times during the day. It may be restricted to a couple of spots in some patients, while some may have widespread rashes appearing for days or even months together. There are acute, sub-acute, chronic and recurring variants as far as the frequency and duration are concerned.

Histopathological view:

Under the microscope, a typical urticarial rash may exhibit perivascular, cellular infiltrate consisting of lymphocytes and eosinophils, is indicative of its allergic behavior. There are findings related to edema (swelling) and mucosal inflammation.

Urticaria: The Inner War:

The urticaria rash is a symptom of an allergic and immunological event taking place at the dermal level. The exact understanding is elusive to an extent. In brief, urticaria is a hypersensitive reaction due to the histamine release. The histamine release could be from the mast cells when antigens and antibodies (IgM or IgG) combine to activate the immunological reaction. The histamine release could IgE induced.
